Note: This Good Deed must be completed before Act 1 ends and before completing Heart of Gold. Going past the deadline will cause Diana Kos's death.

  2. Look for Her Medicine in Vyssa Hills
  3. Return the Medicine to Diana Kos

1. Speak with Diana Kos in the Vyssa Hills Refugee Camp

Hell is Us - Go to past the Timeloop and keep going north to reach the refugee camp

Fast travel to Vyssa Hills and go up to the refugee camp. To get there, go past the first Vyssa Hills Timeloop near the APC and keep going north through the narrow road until you reach a ladder. Climb up, turn right, and speak with Diana Kos upon arriving in the refugee camp. You will find her sitting next to the tents on the right side of the camp.

2. Look for the Medicine Near the Mines' Entrance in Vyssa Hills

Hell is Us - Take the Pill Bottle inside the luggage near the Serpent Ruins
Retrace your steps back to where the Timeloop is located and follow the stone path going down from the entrance of the Serpent Ruin. You will spot a luggage with a pill bottle on the inner side of the path.

Hell is Us - Pick up the Pill Bottle - Atenolol at the side of a hill
Open the luggage to get Pill Bottle - Atenolol. This will activate the Good Deed. Afterwards, go back to the refugee camp.

3. Bring the Medicine to Diana Kos in the Refugee Camp

Hell is Us - Trade the Pill Bottole - Atenolol with Diana Kos

Climb back up to the refugee camp and trade the Pill Bottle - Atenolol with Diana Kos. This will accomplish the Good Deed.

Outcomes Depending on Main Story Progression

Scenario Outcome
If the medicine was delivered to Diana Kos on time She will live and can be spotted in Senedra Forest later on.
If Act 1 ends without delivering the medicine to Diana Kos She will die and her grave will be spotted either in the refugee camp or in Senedra Forest.
If Heart of Gold was completed before delivering the medicine to Diana Kos She will still die and her grave will be spotted either in the refugee camp or in Senedra Forest.

To get the best outcome in Death's Door, deliver the medicine to Diana Kos before completing Heart of Gold and before Act 1 reaches its conclusion. It is also the only way to accomplish the Good Deed, so be mindful not to progress too much into the story if this hasn't been settled yet.

Diana Kos's later location will depend on the outcome in Heart of Gold.

How to Unlock Death's Door

Pick Up the Pill Bottle in the Luggage

Take the Pill Bottle - Atenolol inside the luggage found on the floor at the side of a hill. Trade this with Diana Kos, who needs it to maintain her heart condition.

This Good Deed must be accomplished before Act 1 ends. It should also be prioritized before completing Heart of Gold.
