Frightful Ambush - Base can be found inside a chest in Vitalis' secret underground office in Acasa Marshes. You first need to collect the Family Picture 1, 2, and 3 from inside Vitalis' home to reveal the hidden door in the basement. The code for the hidden door is 321713.

How to Use Glyph - Frightful Ambush - Base
Imbue in Weapons
Frightful Ambush is a Terror Glyph, it can only be socketed into a weapon imbued with the Terror Lymbic Sphere.
This is the only restriction when it comes to Glyphs. You can socket and unsocket Glyphs at any time within the Inventory menu.
